The Degree of Fellow

The Degree of Fellow was established by the American Academy of Urgent Care Medicine to recognize members who have distinguished themselves among their colleagues by their participation in activities outside their medical practice. They have shown themselves to be exemplary physicians dedicated to education, the community and their profession. As a conferred Fellow of the American Academy of Urgent Care Medicine, these honored individuals are entitled to all the honors and privileges bestowed by this degree.

Application Requirements

Any Active or Life Member in good standing may, upon application to the American Academy of Urgent Care Medicine, will be elected to receive the degree of Fellow upon fulfilling the following requirements:

  1. Member must have held Active membership for four years, or held a combination of Resident and Active membership for a total of four years.
  2. Accrue a grand total of 100 points as defined by this application. Cite experiences and activities in the following areas: Academic Training and CME, Publishing and Research, Volunteer Teaching, Public Service, Medical Practice, Service to the Specialty.

Completing the Application

Please provide the information requested in the application. Below each question is a point-count box. Multiply the number of items requested in the question by the factor given and place your total in the box to the right. At the end of each division, a box is provided for your total count for that section. A grand total of 100 points is required to obtain the Degree of Fellow. If you need assistance completing this form, please call 407-521-5789.

Note: Be sure to include only those activities following your residency training through your current activities involvement, unless otherwise specified. If additional space is needed on any part, attach a separate sheet of paper and identify the Division and question number you are completing. In order to insure legibility and to expedite processing, please print or type this application.
